DaVita (NYSE: DVA) announced a value-based care agreement with Humana to provide coordinated care to Humana Medicare Advantage members with chronic kidney disease (CKD) stages 3B–5, according to a company press release.
The partnership, which launched July 1, will serve more than 10,000 patients. It expands on an existing collaboration between the two companies that previously focused on end-stage kidney disease, extending coverage to earlier stages of the condition.
Care will be delivered through DaVita Integrated Kidney Care (DaVita IKC), which coordinates services across cardiovascular, kidney, and metabolic health. The program is designed to stabilize kidney function, reduce hospitalizations, and slow disease progression by engaging patients at an earlier stage.
The model also incorporates support for patients who progress to kidney failure, including education on home dialysis and kidney transplantation, as well as assistance with nutrition, transportation, and mental health needs.
"We are in a therapeutic revolution across kidney care where upstream interventions are helping to reshape patient outcomes," said Dr. Sonia Samagh, national medical director for DaVita IKC. "Through our partnership with Humana, we are deploying advanced care that treats the whole patient, preserves kidney function and serves as a central hub for managing comorbid conditions affecting kidney health."
DaVita states the program builds on its network of approximately 3,000 value-based nephrologist partners. As of June 30, 2026, the company served approximately 298,500 patients across 3,266 outpatient dialysis centers, with 2,671 located in the United States and 595 in 14 other countries.
