The review determined that the type of information potentially impacted, which varies by individual, may include one or more of the following: name, Social Security number, driver's license or state identification number, Medicare or Medicaid number, medical record number, health insurance member number, information relating to diagnosis, treatment, and procedure, medical history or allergies, prescription drugs information, lab test results or images, date of admission or treatment, treatment location, healthcare provider name, cost of treatment, financial account number, street address, phone number, Email address, and date of birth.
In response to this incident, RAC engaged outside cybersecurity specialists to conduct an investigation. RAC also continues to review its policies and procedures related to data protection.
