Iodinated agents are primarily used for X-ray-based imaging services, which include radiography, fluoroscopy, and CT scans. As ContrastConnect explains, the compound contains iodine atoms that react to X-rays, a property that can be used to highlight vascular structures and distinguish them from the surrounding tissues. Iodinated agents can be ionic and non-ionic; the former dissociates into charged particles in solutions and carry a higher risk of adverse reactions, which is why non-ionic agents are more common in clinical practice.

Unlike iodinated agents, GBCAs are often used in MRI due to their ability to alter the local magnetic environment. ContrastConnect’s guide explains that gadolinium shortens the T1 relaxation time of nearby protons, creating a paramagnetic effect that can highlight lesions and vascular structures well; as a result, it is common in neuroimaging, body imaging, and MR angiography. In modern GBCAs, gadolinium ions are bound to chelating agents to prevent the toxic effects of free gadolinium, improving patient safety.

Another form of contrast media is barium sulfate, which ContrastConnect’s guide says is often used for GI tract imaging. These are inert and insoluble compounds that appear radiopaque on X-ray images, as barium effectively absorbs X-rays.

Finally, microbubble contrast agents are gas-filled microspheres stabilized by phospholipid, albumin, or polymer shells that oscillate when exposed to ultrasound waves. Because of their ability to enhance ultrasound signals, they are used in ultrasonography to evaluate liver lesions and cardiac function, among other applications.

All contrast agents require direct supervision by qualified physician or non-physician personnel to ensure patient safety in the event of an adverse reaction. ContrastConnect says that compliant on-site direct supervision can be achieved if supervisory personnel are immediately available and present in the facility. Virtual direct supervision, which is now allowed by the CMS, must be performed through HIPAA- and HITECH-approved technology to comply.
