MHK CareProminence^® and Findhelp integration allows seamless coordination of member health services

The Centers for Medicare & Medicaid Services (2022) highlight how social determinants of health (SDOH) barriers, such as inadequate food or housing, increase the risk of chronic conditions and complicate care management. The CareProminence and Findhelp integration simplifies the process for health plan staff to connect members with essential resources. Member information auto-populates referral and assessment forms, with closed-loop referral data from Findhelp seamlessly syncing back to the CareProminence platform. This streamlined approach allows for tailored member support, catering to individual member needs, and ultimately driving improved health outcomes and overall well-being.
