Chapters Health System Finalizes Affiliation with Hospice of Santa Cruz County

Organization Expands Access to Nonprofit Chronic Illness Care in Central California

TEMPLE TERRACE, Fla., Sept. 30, 2025 /PRNewswire/ -- Chapters Health System, the nation's leading chronic illness innovator and largest nonprofit hospice network, announced the finalization of its affiliation with Hospice of Santa Cruz County (Santa Cruz, CA) today, expanding the Chapters Health West division in California.

Hospice of Santa Cruz County joins Nathan Adelson Hospice (Las Vegas, NV) and Willamette Vital Health (Salem, OR) in the Chapters Health West Division, following the finalization of their affiliations earlier this year. Hospice East Bay (Pleasant Hill, CA) is also expected to finalize its affiliation shortly following the conditional approval granted by California Attorney General Rob Bonta on August 20.
