The pilot, launched at Solera's Lumina Las Vegas Memory Care community, is designed to improve outcomes through coordinated communication and shared data. As part of the model - which would not be possible without the vision of the Las Vegas health care community's renowned Steinberg family - care teams align workflows and hold weekly cross-company clinical calls to provide holistic, resident-centered support and avoid unnecessary ER visits.

Early results are strong: Lumina Las Vegas has seen a 57% reduction in falls, a 54% drop in ER visits, and a 44% decrease in hospitalizations compared to January–June baseline metrics.
