AxisCare Announces VA Assessment and Care Plan Tool for Veteran Care

Built to support providers caring for veterans, the tool helps agencies efficiently document client needs, meet VA documentation requirements, and maintain compliance specifically for those participating in the Homemaker/Home Health Aide (H/HHA) and Respite Care programs. 

The VA Assessment tool will allow home care agencies to record client ADLs, care plan actions, and service details in a format aligned with VA requirements. Once completed, the assessment automatically generates a care plan that is viewable in the caregiver's mobile app. This helps to ensure frontline staff deliver care in alignment with VA expectations. The form can also be exported for upload to the VA's HealthShare Referral Manager (HSRM) or faxed directly to case managers.
