Locally based employers already with Humana health insurance and those businesses seeking alternative choices for health insurance for their employees will find that accountable care agreements reflect a shift away from the current episodic health care model to a value-based model that is designed to lower costs and emphasizes quality and patient-care experiences. Members with HumanaOne individual insurance, both on the marketplace health insurance exchange and off the marketplace exchange, may also seek care through this accountable care arrangement.

“The physicians and staff at Partners in Primary Care have one singular goal – to improve the quality of life of our patients,” said Dr. Bruce Begia, Vice President of Medical Operations for Partners in Primary Care. “Through a value-based Humana partnership approach, our physicians are able to provide wellness, prevention, disease management and a better experience to patients with improved outcomes, like reduction in hospitalizations and emergency room visits.”

Under the agreement, primary care services provided by Partners in Primary Care for Humana commercial members will be available at 10 medical clinics in the surrounding area, including San Antonio, Floresville, Seguin and New Braunfels. Partners in Primary Care already provides primary care services for Humana’s Medicare Advantage members.

“We respect the 20-year history, reputation and clinical expertise that Partners in Primary Care, previously known as Alamo City Medical Group and Allied Institute of Medicine, has in the community,” said Drew Kahn, Regional Vice President of Market Development for Humana. “Their dedication to bringing coordinated, personalized care to patients aligns well with Humana’s goal of improving the health of the communities we serve by 20 percent by 2020.”

The agreement includes value-based incentives tied to performance improvement and supports successful population health management and a world-class patient experience for Humana members and Partners in Primary Care patients in South Texas. The arrangement offers more personalized care and a broad range of population health tools like predictive analytics, chronic care, disease management, care coordination and wellness programs.

The value-based incentive arrangement emphasizes quality outcomes and leverages standard measures defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) such as emergency room visits per 1,000, breast cancer screenings, avoidance of antibiotic treatment in adults with acute bronchitis, and diabetes HbA1c testing.

In addition, physician practices have the opportunity to receive an annual wellness bonus for improvements in their collective Humana patient population. The wellness bonus is a one-of-a-kind opportunity in the marketplace tied to HumanaVitality®, a wellness and rewards program that inspires its members to live healthier lives.

Humana has nearly 30 years of accountable care experience, reflected in results from approximately one million MA members that showed significant improvements in quality, outcomes and costs in 2013. Today, Humana has approximately 1.5 million individual Medicare Advantage and nearly 200,000 commercial members, cared for by approximately 43,000 primary care physicians in more than 900 accountable care relationships across 43 states and Puerto Rico.
