ActiveHealth Management Selects QualityMetric to Measure Health Status

LINCOLN, R.I., Oct. 1 /PRNewswire/ -- ActiveHealth Management (ActiveHealth(R)) is working with QualityMetric to include the SF-12v2 Health Survey in the ActivePHR(SM) personal health record as part of a pilot program. QualityMetric's SF-12v2 Survey is a scientifically valid health status measure that will be available to members of the pilot program who are participants in ActiveHealth's disease management program.

The SF-12v2 Health Survey is one of the most widely used health outcome measures in the world. In just 12 responses, the survey measures general health perception, physical functioning (the ability to climb stairs or carry groceries), bodily pain, physical role limitations, vitality, social functioning (ability to visit friends or relatives), emotional role limitations, and mental health (feeling sad or depressed).
