Lexicon drug shows cardiac and platelet effects in preclinical studies

Lexicon Pharmaceuticals (Nasdaq: LXRX) announced findings from two preclinical studies examining the biological effects of sotagliflozin, its dual SGLT1 and SGLT2 inhibitor, in cardiovascular disease models.

The first study, published in the Journal of Molecular and Cellular Cardiology Plus, used a mouse model of hypertrophic cardiomyopathy (HCM) and found that sotagliflozin normalized diastolic function, reversed cardiac hypertrophy progression, reduced cardiac fibrosis, and reduced oxidative stress while upregulating mitochondrial gene sets associated with energy production in heart tissue.

The second study, published in the Journal of Clinical Investigation, found that sotagliflozin reduced platelet activation and thrombus formation in ex vivo human blood and animal models without adversely affecting normal hemostasis. The selective SGLT2 inhibitor empagliflozin did not produce the same effects, with investigators attributing the difference to sotagliflozin's additional inhibition of SGLT1.

"These studies add to a growing body of evidence demonstrating the broad biological effects of sotagliflozin across multiple pathways relevant to cardiovascular disease," said Craig Granowitz, M.D., Ph.D., chief medical officer of Lexicon.

Both studies were investigator-initiated and funded by Lexicon. The company said the platelet findings may provide context for previously observed reductions in myocardial infarction and stroke risk associated with sotagliflozin in heart failure and type 2 diabetes clinical trials.

Lexicon is currently conducting the Phase 3 SONATA-HCM trial evaluating sotagliflozin in patients with symptomatic obstructive and non-obstructive hypertrophic cardiomyopathy. The company expects topline results in the first quarter of 2027.
