Humana and Northwest Physicians Network Form Accountable Care Partnership to Improve Care for Washington Seniors

Agreement is designed to increase access to care and improve overall health and well-being for Humana Medicare Advantage members

SEATTLE--(BUSINESS WIRE)-- Northwest Physicians Network, LLC (NPN), a Washington state independent physicians association, and Humana Inc. (NYSE: HUM), one of the country’s leading health and well-being companies, today announced an Accountable Care agreement that will provide Humana Medicare Advantage members access to NPN’s more than 500 providers and 200 ancillary facilities throughout the Puget Sound region.

Humana recognizes the need to marry a physician’s clinical expertise with business savvy to drive a cohesive integrated care approach. By partnering together with NPN on accountability, care moves from an episodic to a value-based model that improves costs, quality and the patient experience.

Northwest Physicians Network serves community-based providers in Washington state by providing access to patients, tools, and services which enable each practice to function within a high-performing clinical network bringing value to health plans, providers, and patients alike.

The agreement with NPN is part of Humana’s value-based model that promotes evidence-based, high-quality care with a focus on physician support for successful population health management. The value-based model is designed to improve cost, quality and the patient experience.

The new Humana-NPN accountable care agreement includes value-based incentives tied to performance improvement and supports successful population health management and a world-class patient experience for Humana members and NPN patients in Washington. The arrangement also aligns NPN and Humana’s complementary population health capabilities, including predictive analytics, chronic care, disease management and wellness programs.

Through its Accountable Care partnerships, Humana gathers actionable patient data to inform individual care plans and develop best practices in patient care. Value-based incentive arrangements emphasize quality outcomes and leverage standard measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS), such as breast cancer screening, colorectal screening, comprehensive diabetes care, and high-risk medications.

Humana has nearly 30 years of accountable care experience, reflected in results from approximately one million Medicare Advantage members that showed significant improvements in quality, outcomes and costs in 2013.
