Explanation of Responses:
1. The number of shares acquired and the number of shares beneficially owned following the reported transaction have been adjusted to reflect the one for four reverse split of the common stock effective May 18, 2016.
2. These shares were received in exchange for the issuer's 10% senior notes 2017 in an issuer exchange offer.
3. These amounts were erroneously reported as 89,213 and 90,180 shares, respectively, on the original Form 4.
4. These shares were received in exchange for the issuer's 10% second lien senior secured notes due 2021 in an issuer exchange offer.
5. Previously reported as being held by Delaware Charter FBO Wayne Nordberg R/O IRA. In addition, the reporting person no longer has a reportable beneficial interest in 19,988 shares owned by the Anna & Samuel Nordberg Trust and included in the reporting person's prior ownership reports
6. The reporting person no longer has a reportable beneficial interest in 15 shares owned by Janet Mavec Custodian FBO Akash Mavec, UBMA.
