Authorities stated that the 44-year-old repairman somehow became stuck between the elevator door and the top portion of the elevator car and was dragged upwards and killed.

The worker was said to have been found wedged between an elevator cab and the wall of the shaft, according to the federal Occupational Health and Safety Administration and was pronounced dead at the scene. Initial reports stated that the job was normally performed by two workers but that the man had begun work when his partner was late. Slade Industries, Inc., the worker's employer, expressed it's condolences to the family. OSHA is investigating the death to determine any wrongdoing.
