Broward Health to Expand Cerner Services With Population Health Management

FORT LAUDERDALE, Fla., Dec. 2, 2015 (GLOBE NEWSWIRE) -- Broward Health, a nationally recognized health system, has announced it has selected Cerner, a global leader in health care technology, to implement population health management solutions. Cerner, the health system's electronic health record (EHR) supplier, will implement HealtheIntentsm, a system-agnostic, near real-time population health management platform designed to accept and normalize data from disparate sources.

Broward Health care providers will have a more comprehensive view of their patient population, including EHR, claims, pharmacy, lab data and more, which will enable them to make more informed care decisions. Broward Health will expand its patient engagement options and implement HealtheLife: Engagementsm. This extended solution provides tailored health and wellness tools, such as custom education and health-tracking tools, for individuals to engage in their personal health and care.

Broward Health will also implement Cerner's Readmission Prevention solution, a multidisciplinary solution, to help predict and identify patients at risk for readmissions, facilitate processes across facilities and maintain a unified workflow. The Cerner all cause readmission model, the identification component of the solution, outperforms other peer-reviewed prediction models.
